Years after Saw III (2006) and Jigsaw's death, pathologist Logan Nelson and forensic assistant Eleanor Bonneville investigate the mutilated remains of what seems to be the latest victim of the long-deceased criminal mastermind. In the meantime, Detectives Halloran and Hunt are on the trail of a ghost from the past as five sinners find themselves trapped in a deadly game designed to beat a confession out of them.
